ARIZONA
If you feel that taking a step back from gambling is the right choice for your well-being, we fully support you. In Arizona, you can request a permanent break by enrolling in the Arizona Self-Exclusion Program, which allows you to self-exclude for 1, 5, or 10 years. During this period, your Bally Bet account will remain closed and cannot be reopened under any circumstances.
How to Enroll:

- Have the form notarized- Notary appointments are required to ensure availability.

COLORADO
If you feel it’s time to take a meaningful break from gambling, we support your decision. In Colorado, you can voluntarily self-exclude from all gaming regulated by the Colorado Department of Revenue for 1, 3, or 5 years.

Voluntary Self-Exclusion Program (SEP) Overview
The Illinois Gaming Board (IGB) offers a Voluntary Self-Exclusion Program (SEP) for individuals who want to restrict their access to gambling within the state. Anyone enrolled in the Voluntary Self-Exclusion Program is prohibited from entering Illinois casinos and from participating in sports wagering activities.

MASSACHUSSETTS
If you feel the need to take a break, you can self-exclude for 1 year, 3 years, or 5 years by choosing one of the following convenient options:
Option 1: Massachusetts Voluntary Self-Exclusion (MA VSE) Program
This program is managed by the Massachusetts Gaming Commission and applies to all licensed operators in Massachusetts.

- The self-exclusion period is a minimum of 2 years or Lifetime.

You may enroll online or schedule an in-person appointment at the PGCB’s Harrisburg office or at a licensed Pennsylvania casino.

Once enrolled, you will be prohibited from accessing all online gambling platforms in Tennessee for the duration of the selected term.
