ARIZONA
If you feel that taking a step back from gambling is the right choice for your well-being, we fully support you. In Arizona, you can request a permanent break by enrolling in the Arizona Self-Exclusion Program, which allows you to self-exclude for 1, 5, or 10 years. During this period, your Bally Bet account will remain closed and cannot be reopened under any circumstances.
How to Enroll:

- Have the form notarized- Notary appointments are required to ensure availability.

Include a current color photo- Attach a clear, original photo (minimum 2" x 3") showing your head and shoulders. A copy of a driver’s license is not accepted.

COLORADO
If you feel it’s time to take a meaningful break from gambling, we support your decision. In Colorado, you can voluntarily self-exclude from all gaming regulated by the Colorado Department of Revenue for 1, 3, or 5 years.
Once enrolled in the Colorado Self-Exclusion Program, your Bally Bet account will be closed and cannot be reopened during the exclusion period.

IOWA
If you feel it’s time to take a longer or permanent step back from gambling, the Iowa Racing & Gaming Commission (IRGC) offers a Self-Exclusion Program for 5 years or a lifetime.
Once enrolled, your Bally Bet account will be closed and cannot be reopened during your exclusion period. This also applies across all websites operated by Bally’s.

How to Enroll in the Voluntary Exclusion Program (VEP):

Please Note:
- The self-exclusion period is a minimum of 2 years or Lifetime.
No individual on the list may request removal until two years have passed from the date of placement. - For requests to be removed from the list after the exclusion period, individuals must contact the Maryland Lottery and Gaming Commission directly to understand and complete the required steps.

If you wish to voluntarily exclude yourself from online gaming in New Jersey, you may enroll in the New Jersey Casino Gambling Self-Exclusion Program for a period of 1 year, 5 years, or lifetime (lifetime exclusions must be completed in person).

PENNSYLVANIA
If you’d like to voluntarily exclude access to your online account, you can enroll in the Pennsylvania Gaming Control Board (PGCB) Self-Exclusion Program for a period of one (1) year, five (5) years, or lifetime. You may enroll online or schedule an in-person appointment at the PGCB’s Harrisburg office or at a licensed Pennsylvania casino.

Once enrolled, you will be prohibited from accessing all online gambling platforms in Tennessee for the duration of the selected term.
